Nursery Request Form

We love to take care of your little ones while you are in service to the Lord. To ensure their safety we need to have enough adults working. AS SOON AS YOU SET UP AN EVENT, please alert us by filling out this form and returning it to us, (no later than 2 weeks prior). The number of children should be given to us no later than one week before the event. These numbers are essential in scheduling our workers.
